Goal
To reduce cardiovascular and renal complications for patients with diabetes in Simcoe County and beyond
Patients with diabetes and existing cardiac or renal disease
unable to be started on appropriate meds (Eg, on insulin/multiple meds/ co-morbid diseases/initial side effects that limit ability to be initiated on meds)
Focused multi-disciplinary intervention over 12 months with General Internists, pharmacists and diabetic educators with f/u through the referring physician/primary care provider.
(not a long-term chronic disease management program).
